由于美国陆军对柯尔特XM148实战试验并不成功，为此，美国陆军武器研究部门在1967年7月宣布了一项名为GLAD的研究计划，这是“榴弹发射器附件研究”（Grenade Launcher Attachment Development）的缩写，该项目明确要求发展一种代替XM148的榴弹发射器。这次邀请了17家轻武器公司参加GLAD投标，只有7家表示了参与的兴趣，在1967年9月又参与投标的公司中选出了3家公司继续对比，分别为：Philco-Ford公司，Aero Jet General公司，和AAI公司（前Aircraft Armaments, Inc.）。Aero Jet General的原型是改进的SPIW式半自动发射器，而Philco-Ford则是枪管侧面摆动装填的单发型枪挂式榴弹发射器，AAI的设计也是基于他们参与SPIW项目的样品，是枪管滑动式装填的单发型枪挂式榴弹发射器，而柯尔特的样品没有入围。

经过对比试验后，1968年11月，美国陆军决定试用AAI的榴弹发射器，并命名为XM203。12月，600具XM203送到越南进行实战测试。1969年，美国陆军又在阿泊汀试验场和本宁堡进一步试验和改进XM203。XM203的研制报告在 1969年9月提交。经过少量改进后，XM203在1970年8月被正式命名为M203。但讽刺的是，柯尔特自己的榴弹发射器在试验中失败了，陆军却因为采购价格的问题，把所有M203的生产合同都判给了柯尔特公司，柯尔特公司共生产了数十万具M203，而AAI只生产了最初用于实战试验的600具，此后许多其他公司所生产的M203仿制型大部分都是由柯尔特公司所授权的。不过无论如何，M203使M16成了第一种点面杀伤相结合的武器，就像M16本身带动了小口径突击步枪的潮流一样，M203枪挂式榴弹发射器的方式也被其他国家相继仿效。

当M203挂在M16/M4的枪管下时，射击时要利用弹匣体作为握把。M203可发射高爆弹、烟雾弹、照明弹、霰弹、CS毒气弹和训练弹。在美国或其他国家生产特许生产的M203有多种型号，有些枪管长度不同，有些是附挂的形式不同，其中一些具有快速拆卸能力。

标准的M203通常安装在M16A1、M16A2、M16A3步枪上，为12英寸（约305mm）长的线膛枪管，也能安装到M4/M4A1卡宾枪上，但SOPMOD套件中使用的榴弹发射器是M203A1。M203A1是专为加挂到M4/M4A1上使用的，其枪管缩短到9英寸（约228mm），而且其中的M203A1 QD型可快速从步枪上拆卸，适用于USSOCOM的SOPMOD M4或陆军的M4 MWS，可与KAC的M4 RAS的下护木互换，其瞄准具通过M1913导轨安装在步枪上。新的M203A2是用于M16A4 MWS系统的，采用标准的12英寸枪管，可与KAC M5 RAS的下护木互换，因此同样具有快速拆卸功能。

M203在2005年时的重置单价为601美元。
